This Service Order is subject to and governed by the Blink Voice Communications, Inc. Managed Services Agreement (“MSA”), and any Statement of Work (“SOW”), Terms of Services, Service Level Agreements and the various services and equipment policies (collectively “TOS”) documents for Blink Voice Services which are available at https://www.blinkvoice.com/legal.  (the MSA, SOW and TOS collectively with this Quote, the “Agreement”). Your signature above acknowledges that you have read, understand the TOS and accept the terms and conditions of the Agreement and that you are duly authorized to execute and deliver this Quote. The Agreement is incorporated by reference into this Service Order and by signing this Quote, the customer agrees to the Agreement. Unless otherwise defined herein, capitalized terms used herein shall have the meaning ascribed to them in the Agreement. The Agreement constitutes the entire agreement between the Parties and supersedes all prior agreements (written or oral), understandings, statements or proposals concerning the Services. No statement, representation or warranty made by any agent or representative of Blink Voice regarding the Services to be provided by or the rates therefore shall be binding upon Blink Voice unless expressly included herein. By signature above Customer promises to timely pay Blink Voice all fees and charges set forth above under the Agreement and any late fees or interest thereon.

 

This Service Order shall not become valid until an authorized representative of Customer and Blink Voice has executed this Service Order or the order has been placed. This Service Order is not binding without a Blink Voice authorized signature. The Parties hereby acknowledge that the Service Order may be executed in two or more counterparts and duplicate originals, including electronic and facsimile counterparts, each of which shall be deemed an original, but all of which shall together constitute one and the same instrument. This Service Order shall be held as Confidential Information by the Parties.
